Spring Child & Adult Psychiatry is dedicated to providing you with the highest quality, cost effective mental health care. We accept all major credit cards, cash, and check.  There is a $25 fee for all returned checks.  Payment or insurance co-pay is due at time of service.

INSURANCE & PAYMENTS

Insurance: 

 

Medication Management and Therapy Services may be covered in full or in part by your health insurance.

You are responsible for verifying your mental health benefits for therapy.   You are also responsible for any balance due for services, including situations in which you have not met your insurance deductible. Please carefully check your coverage by asking the following questions:

  • Do I have outpatient mental health insurance benefits?

  • What is my deductible and has it been met?

  • How does meeting my deductible influence my mental health coverage?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session?

  • Is Spring Child & Adult Psychiatry an in-network provider for my insurance plan?

 

We accept the following insurance plans:

  • Soonercare (Medicaid) - Ages 5 to17 years old only

  • Blue Cross/Blue Shield*

  • Health Choice*

  • For any other insurances, we will provide the information you need to file with your insurance company yourself, but your appointment will be treated as self-pay with our office.

*To schedule a new patient appointment, a $125 deposit is required.  This deposit will be kept until the first Explanation of Benefits (EOB) is received from your insurance provider and will be applied to the initial assessment fee if needed.  Refund of deposit is available if appointment is cancelled 7 days in advance.

Private Pay

 

If you are uninsured or would prefer your services not be filed with an insurance company, private pay rates are available. PAYMENT IS DUE AT TIME OF SERVICE.

RATES:

  • Initial Assessment $295.00

  • Follow-up $140.00

Additional Fees: 

 

There is an additional $50 fee for completion of forms for use outside of the patient's medical record, including:

  • Day Care

  • Social Security

  • Life Insurance

  • Family Medical Leave

Cancellation Policy

 

24 hours notice is required to change or cancel an appointment. Failure to do so will result in a $50 fee.
